Informed Consent and Acknowledgement

1. Voluntary Participation

   I, the undersigned, am voluntarily participating in martial arts and Brazilian Jiu Jitsu training and activities at Platinum Jiu Jitsu. I understand that these activities include, but are not limited to, physical training, sparring, and competitions.

2. Assumption of Risk

   I acknowledge and understand that martial arts and Brazilian Jiu Jitsu are physically demanding activities that carry inherent risks, including but not limited to physical injury, disability, or death. These risks may result from my own actions, inactions, or negligence, or the actions, inactions, or negligence of others, including but not limited to Platinum Jiu Jitsu staff, instructors, and other participants.

3. Medical Clearance

   I confirm that I am physically fit and have no medical condition that would prevent my full participation in these activities. I will inform the staff of any health conditions or injuries that may affect my ability to participate.

Waiver and Release of Liability

1. Waiver of Liability

   In consideration of my participation in the activities offered by Platinum Jiu Jitsu, I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, waive, and discharge Platinum Jiu Jitsu, its directors, officers, employees, instructors, agents, and other participants from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in these activities or while on the premises.

2. Indemnification

   I agree to indemnify and hold harmless Platinum Jiu Jitsu, its directors, officers, employees, instructors, agents, and other participants from any and all claims, demands, damages, expenses, or causes of action, including reasonable legal fees, arising out of or in connection with my participation in the activities or my use of the facilities.

Consent to Be Filmed and Use of Images and Videos

1. I consent to being filmed, photographed, or recorded during my participation in activities at Platinum Jiu Jitsu. I understand that these recordings may capture my likeness, voice, and performance.

2. Use of Images and Videos

   I grant Platinum Jiu Jitsu the right to use, reproduce, and distribute any photographs, videos, or other recordings of me for promotional, marketing, and educational purposes, including but not limited to social media, websites, and other digital platforms.

3. Waiver of Rights

   I waive any right to royalties or other compensation arising from or related to the use of these recordings. I also waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears.

Informed Consent and Acknowledgement

This waiver and release shall be governed by and construed in accordance with the laws of the State of Queensland, Australia, without regard to its conflict of law principles.

Severability

If any provision of this waiver and release is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary so that the remaining provisions shall otherwise remain in full force and effect.

Acknowledgement of Understanding

I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
